Community Based In-Home Services helps seniors
Rendezvous Pointe helps people maintain their independence

Sublette County Seniors age 60 and over can benefit from a program called Community Based In-Home Services, offered by the State of Wyoming Department of Health and Aging Division. It provides personal care and homemaking services to seniors, their spouses, and some disabled individuals 18 years of age or over.
Services include homemaking, personal care and other support. In Sublette County, the program is offered through Rendezvous Pointe on a fee scale based on the person’s ability to pay fee. Potentially eligible individuals participate in a needs assessment to determine if they qualify for the program. The assessment is used to develop the care plan. The client has the opportunity to approve the hourly amount suggested. Client’s are not denied services if they are unable to pay. Each client is assessed quarterly to see if the needs have changed or services increased or decreased.
Homemaking services include light housework, shopping, essential errands, meal preparation, correspondence, helping to pay bills, and other home chore services. Caregivers can also assist with bathing, dressing, hair care and personal hygiene tasks. Other assistance can include minor home modification, temporary respite, installation and maintenance of life lines.
The goal is to help Sublette County seniors and disabled adults maintain their independence and live in their own homes for as long and as safely as possible.
